오늘은 Risk stratification 을 좀 뜯어보려고 해요.

어우 복잡합니다.

8개의 Risk Group 을 하나 하나씩 살펴볼게요

 

Very low

All of the following:            4가지 조건을 모두 충족해야 함

• T1c
• Gleason score <= 6 / grade group 1     글리슨 점수 6 이하와 글리슨 등급 1은 같은 의미입니다.
• PSA < 10 ng/mL
• <3 prostate biopsy fragments/cores positive, <= 50% cancer in each fragments/core

          전립선 생검 조각 3개 미만 또는 모든 core 양성비율이 50% 이하

• PSA density < 0.15 ng/mL/g          PSA 농도 0.15 미만

 

※ 글리슨 점수와 글리슨 등급 체계

글리슨 등급 (GG) 분류 체계 (2016 WHO grading system)

 

 

Low

All of the following:            3가지 조건을 모두 충족해야 함

• T1-T2a
• Gleason score <= 6 / grade group 1
• PSA < 10 ng/mL

       ※ Very low 와 꽤 비슷하네요. Very low 의 마지막 4~5번째 조건은 빠지고 T stage 는 다음 단계입니다.

Intermediate-favorable

Any of the following:          다음 중 아무거나 하나

• T2b-T2c
• Gleason score 3+4=7 / grade group 2      ※ 동일한 GS 7점인 경우에도 3+4 와 4+3은 등급이 달라요
• PSA 10-20 ng/mL

     PLUS percentage of positive biopsy cores < 50%        추가로, 양성인 코어의 비율이 50% 미만

     조금 어렵네요. PLUS 라는 것이 위 3가지와 별개로 반드시 필요하다는 의미인 것 같은데...

 

Intermediate-unfavorable

Any of the following:         다음 중 아무거나 하나

• T2b-T2c
• Gleason score 3+4=7 / grade group 2 or Gleason score 4+3=7 / grade group 3
• PSA 10-20 ng/mL

     ※ Intermediate-favorable 과 거의 똑 같아요.  or Gleason score 4+3=7 만 추가되어 있어요

 

High

Any of the following:          다음 중 아무거나 하나

 

• T3a
• Gleason score 8 / grade group 4 or Gleason score 4+5=9 / grade group 5
• PSA > 20 ng/mL

 

Very high

Any of the following:          다음 중 아무거나 하나

• T3b-T4
• Primary Gleason pattern 5            GS 5+아무거나
• >4 cores with Gleason core 8-10 / grade group 4 or 5          조건 참 복잡하네요

grade group 으로 보아 Gleason core 는 Gleason score 오타가 명백하네요
GS 8~10 으로 측정되는 코어가 4개 초과라는 뜻인가 봐요

 

Regional

Any T, N1, M0             원격전이가 안된 상태(M0)에서서는 다른거 볼거 거 없이 림프절 전이(N1)가 있으면  이거라네요

 

 

Metastatic

Any T, N, M1            M1 즉 원격전이 단계이면 다른거 보지도 말고 그냥 이거라네요
